Sometimes people just need help getting better over a period of time. Perhaps they’ve sustained an injury through their daily living activities, or their hip has finally worn out, or they are learning to live with a new treatment.

For others, addressing a chronic disease like diabetes, people need help managing their illness so they can participate in life to the greatest extent possible and avoid hospitalization.

WHAT IS HOME HEALTH CARE?

Home Health care encompasses a wide range of part-time/intermittent services in the areas of nursing and/or physical therapy.

Home Health meets the needs of two distinct groups: those who are in the process recovering from a specific medical procedure such as a hip replacement and need specific care during their recovery; and those who have a chronic disease which requires intermittent skilled care on an ongoing basis in order to avoid hospitalization.

A treatment plan for each patient is developed with the referring physician.

Home health care is a proven alternative to institutional care. Numerous studies have documented the fact that (depending on the situation) people recover faster and live longer when treated in their own home.

HOW DO I QUALIFY FOR HOME HEALTH CARE?

Medicare and Medicaid are both government programs. Each has its own rules and regulations that determine who is qualified to receive care, how much care an individual is entitled to, and how that care is to be provided.

To qualify for the Medicare Home Health Care Services, you must begin by getting a referral from your physician. Mountain Home Health Care clinical staff, with your physician, will determine the skilled care needs and homebound qualification according to Medicare criteria.

WHO PAYS FOR HOME HEALTH CARE?

For qualifying individuals, Home Health Care may be covered under Medicaid or Medicare. In other cases, it is covered by private insurance.

Home Health Care includes three major categories of service.

NURSING

  • Teaching/Management of Medication
  • Wound Care
  • Infusion Therapy
  • Diabetic Care/Teaching
  • Self Care Encouragement
  • Check vital signs (02 Saturation)
  • Catheter Care/Change
  • Coumadin (monitor)
  • Lab Work
  • Enteral Feeding (tube-feed) & Education to Family

PHYSICAL THERAPY

  • Exercise Strengthening
  • Gait (walking) Training
  • Assist with obtaining Hoyer & Educate Family (wheelchairs/walkers)

HOME HEALTH AIDE

  • Bathing/Hygiene
  • Catheter Care
  • Minimal Housekeeping

WHO DELIVERS THE CARE?

Home Health Care is delivered by the appropriate combination of staff Registered Nurses, Physical Therapists and Assistants, and Home Health Aides.
